the only insurance plan our office participates in is Cigna Dental DPPO. if you are a member of cigna dental Dppo, you will be responsible for any copays, deductibles, and charges not covered by your dental insurance plan at time of service. unlike medical insurance, dental insurance does not cover all services, and it has a yearly maximum payout based on the contract your employer negotiated with cigna. this office is not in network with cigna dental hmo or dhmo,
Aside from Cigna Dental ppo, Our office does not participate with any dental insurance plans. Therefore, payment in full is due at the time of your service. You may still be covered by your dental insurance plan when you visit our office, if you are permitted to see an "Out of Network" provider, or have a PPO type dental insurance plan. Dental Insurance is a contract between your employer and an insurance company. The benefits you receive are based on the terms of the contract your employer negotiates. Most dental insurance policies provide only for basic care and treatment. The amount of coverage varies, with a typical range of 50-80 percent of the total charges for basic and major dental care, and sometimes 100% for preventative and diagnostic care, up to your yearly limit. Our dental office did not participate in the negotiations; therefore, we have no influence as to how much, if any, coverage your policy provides. Many of the services necessary to achieve high quality, the complete result are not necessarily covered. Check out your plan specifics or call your insurance carrier for benefits information, as all dental plans are different, even within the same company.
Insurance Claim Forms: As a courtesy, we will submit your insurance claims at the end of your appointment should you request us to do so. If not, it is the patient's responsibility to file their claims. Once Insurance claim forms are submitted to the patient's insurance carrier for payment, payment is issued and sent directly to the patient (subscriber). This should take around 2-4 weeks from date of service.
aside from Cigna dental ppo, Our office is a "Fee-for-Service" practice, which means we require payment, in full at the time of service, to be made to Edward A. Alvarez, DDS, PC. We currently accept Visa, Mastercard, American Express, Discover, as well as checks and cash.
Patient financing is available through Lending Club or Care Credit.
